Horizon Health believes everyone plays an important role in helping meet patient needs. We realize that meeting those patients means including family members in the conversation.

The Conversation Needs You

Our goal is to bring the hospital and community together when making decisions about patient care. To begin that conversation, the Patient Family Advocacy Council has been created and needs you. Note that your initial commitment of time is minimal.

Become a Member

As a member of the Patient Family Advocacy Council you will help us accomplish the following:

  • Ensure patient and family centered  care
  • Improve teamwork between caregivers, patients, and families
  • Build a positive relationship between the hospital and community to improve patient and family satisfaction
  • Have direct input on the hospital’s priorities and planning efforts

The Patient Family Advocacy Council's core concepts are:

Respect

We value a family’s knowledge of their loved ones; recognize their authority as decision makers; and respect their choices, family values, and beliefs.

Collaboration and Partnership

We work together to best meet the needs of families and their loved ones.

Participation

Patients and family members participate in care and care planning. Staff prepares and supports those patients and families to participate at a level they choose.
