Medicaid Resources

Horizon Health offers resources to help you apply for or renew your Illinois Medicaid. Other resources can help you apply for and manage your food assistance, cash benefits, and more.

What you need to apply for Medicaid
(by patient category)

Disabled or 65 (More detailed because assets are considered)
  • Driver’s license(s)
  • Birth certificate(s)
  • Social Security card(s)
  • Copy of Medicare card(s), if applicable
  • Life insurance policies: cash value must be included
  • Copy of deed and title to your home
  • Copy of registration(s) to all vehicles
  • Bank statements (checking, savings, etc.) for last three months
  • Proof of cash values of any assets (annuities, retirement plan, etc.)
  • Copy of any Social Security benefits award letter(s) showing the current year’s benefit amount(s)
    • If other monthly income, such as a pension, that documentation will be required as well
  • Last 3 months of gross income (before taxes)
  • Copies of insurance premiums paid by you directly (supplemental coverage)
  • Copies of all paid and unpaid medical bills from all facilities, including prescriptions, for the last year
Families with children: US Citizenship is NOT required for children only
  • Driver’s license for both parents
  • Birth certificates for all family members
  • Social Security cards for all family members
  • If insured, copies of insurance cards for each family member
  • Last 3 months gross income (before taxes), taxable and non-taxable (check stubs from employment, child support, Social Security, etc.)
  • Letters of assistance if monetary assistance is received (signed statement with details of assistance provided-dates, amounts, purpose of payment, such as rent, car payment, car insurance, etc.)
  • Self-employed: Full tax return for latest year, including profit and loss statement
Medicaid Expansion (Single or couples without children)
  • Income based- 138% of Federal Poverty Level
    2019 income limits
    • Single- $1,436.35/month or $17,236.20/year
    • Couple- $1,944.65/month or $23,335.80/year
  • Driver’s license(s)
  • Birth certificate(s)
  • Last 3 months gross income (before taxes)
  • Letters of assistance if monetary assistance is received (signed statement with details of assistance provided-dates, amounts, purpose of payment, such as rent, car payment, car insurance, etc.)
