Horizon Health Maintains National Quality Recognition

For the second consecutive year, Horizon Health has earned the American Heart Association (AHA) Commitment to Quality Award, recognizing excellence in cardiovascular and stroke care. Horizon Health is one of only 222 sites nationwide to receive this prestigious honor, which was first established in 2025.

“Receiving this award for a second consecutive year demonstrates our ongoing commitment to providing the highest quality of care and achieving the best possible outcomes for our patients,” said Jay Carmichael, Horizon Health President & CEO. “This recognition is a direct reflection of the dedication, expertise and teamwork of our staff. Their commitment to following evidence-based practices and delivering exceptional care makes achievements like this possible.”

To qualify for the Commitment to Quality Award, Horizon Health demonstrated high performance in stroke, heart failure, and coronary artery disease care through the AHA’s Get With The Guidelines program. Get With The Guidelines is a voluntary quality-improvement initiative that helps hospitals enhance patient care and outcomes in the communities they serve by applying the latest research-based treatment guidelines.

In addition to the Commitment to Quality Award, Horizon Health received six other excellence awards through the Get With The Guidelines program:

Note: Non ST-elevation acute coronary syndrome (NSTE-ACS) refers to patients experiencing unstable chest pain. ST elevation myocardial infarction (STEMI) refers to patients experiencing a very severe and dangerous type of heart attack.

  • Coronary Artery Disease STEMI Referring: Silver Plus Award
  • Rural Coronary Artery Disease NSTE-ACS: Gold Award
  • Rural Coronary Artery Disease STEMI: Gold Award
  • Rural Heart Failure: Gold Award
  • Heart Failure: Silver Plus Award

Horizon Health Emergency Medical Services also received the AHA’s Mission: Lifeline EMS Gold Award, which is separate from the Get With The Guidelines program.

Joynt Maddox, MD, MPH, chair of the American Heart Association Quality Oversight Committee. “By taking a systems-level approach to quality, these hospitals are building a strong foundation for better outcomes, fewer complications and healthier communities."
