Patient Portal

Access to Your Health

We are happy to offer our patients access to their health information through the online patient portal. Whether you are looking to schedule an appointment, request your medical records or view test results, you can have access wherever you are and whenever you need.

If you are accessing the portal for the first time on a mobile device, please select the "Full Site" link. Then click on the "Sign up today" link to complete your registration.

Patient Portal Login

If you would like to allow another individual to access your patient portal-- such as a parent (of children age 12 or older), legal guardian, or power of attorney--please fill out the online form. Or, print a physical form here . Be sure to include your signature. Bring the form to the registration desk at either Paris Community Hospital or the Paris Clinic with a photo ID to have “family access” granted.

Welcome to Your Patient Portal!

Within the portal, you will be able to:

  • View and update your medical history, allergy, and medication lists
  • Send secure messages to your provider
  • Request an appointment/View scheduled appointments
  • View test and lab results
  • View information related to your hospital/inpatient stay

Patient Portal Registration

If your Patient Portal email is not already registered click here. Once directed to the Patient Portal Interest Form please insert your information and click submit. Your Patient Portal account will be created for you and linked to your records within a couple of days.

(The email address used to create your portal account should be the same email address that you provided to our registration staff during the intake process.)

(Note that you must be an established patient at Horizon Health to create a Patient Portal account.)

What information is available on the Patient Portal?

Health information at the Horizon Health clinics (as of Jan. 12, 2016), as well as inpatient/hospital information (as of April 11, 2017), is available on the Patient Portal.

To request medical record information outside of the Patient Portal:

  • Click here to request records online
  • Clinics (Paris, Chrisman, Oakland), call (217) 466-4514.
  • Paris Community Hospital, call (217) 466-4209.
