
We are happy to offer our patients access to their health information through the online patient portal. Whether you are looking to schedule an appointment, request your medical records or view test results, you can have access wherever you are and whenever you need.

If you are accessing the portal for the first time on a mobile device, please select the "Full Site" link. Then click on the "Sign up today" link to complete your registration.
If you would like to allow another individual to access your patient portal-- such as a parent (of children age 12 or older), legal guardian, or power of attorney--please print and complete the form here. Be sure to include your signature. Bring the form to the registration desk at either Paris Community Hospital or the Paris Clinic with a photo ID to have “family access” granted.

Not registered? No problem! Just click the "Log in" link. Once directed to the Portal, you will be given the option to create a new account. The patient portal provides information about medical history, contact information, payment history and more. Any information that isn’t available online can be available upon request from the medical staff.
Note that you must be an established patient at Horizon Health to create a Patient Portal account.
Health information at the Horizon Health clinics (as of Jan. 12, 2016), as well as inpatient/hospital information (as of April 11, 2017), is available on the Patient Portal.
