Senior Care

Comprehensive Health Services for Seniors

The Horizon Health Senior Care team provides high-quality assistance for seniors dealing with life’s many challenges. Our medical needs increase as we get older, and it’s important to have a medical team that is familiar with these needs and will support you or your loved one. Your first visit to Senior Care will consist of a physical and mental health evaluation, medical history review, and discussion of treatment options going forward. The Senior Care team is dedicated to providing exceptional care for seniors.

We provide the following Senior Care services:

  • Group therapy
  • Individual therapy
  • Family counseling
  • Health assessment
  • Medication management
  • Fall prevention
  • Home modifications
  • Health and wellness training

Going Beyond Physical Care

While seniors have many physical health needs, it would be a mistake to overlook their mental health. Getting older comes with many stressful challenges such as witnessing passing, leaving one’s own home to live in an assisted care facility, and dealing with your own increasing medical needs. At Horizon Health, we take a holistic approach to senior care, taking the time to address a patient’s physical, mental, and emotional needs.

Conditions we treat include:

  • Depression
  • Unresolved grief
  • Anxiety
  • Isolation
  • Anger
  • Difficulty coping with health/physical challenges
  • Feelings of hopelessness or helplessness
  • Worry
