If you are recovering at home from an illness or injury, Horizon Health
Home Care can help. Our team of nurses, therapists, and aides provide
home-based health services that assist you with recovery, regaining independence,
and becoming as self-sufficient as possible.
Along with skilled nursing care, we provide other skilled care services,
including physical, occupational, and speech therapy. Our home health
aides can assist you with personal care needs, including bathing and meal
preparation.
A provider’s order is needed to participate in all Home Care services.

Coverage Options
Our Patient Services team works on your behalf to help determine your insurance
coverage for home health. Payment sources include:
- Private insurance
- Private cash pay
- HMOs and PPOs
- Medicare (See below)
- Medicaid
- Workers’ compensation
Medicare Criteria
Our team works with you and your family to ensure the criteria below is
met for Medicare coverage:
- People must require services authorized by their doctors and coordinated
within a plan of care
- People must need skilled services that are reasonable and necessary for
treatment of their illnesses or injuries
- Services identified must only be needed on a part-time and intermittent basis
- People will be assessed by a registered nurse to determine homebound status
