About the provider

Amy joined Horizon Health in January 2024, supporting the practices of Drs. Yaniv Cozacov and Fouad Hachem, general surgeons who specialize in bariatric surgery. She provide pre-operative and post-operative care for patients having general surgery, bariatric surgery, and gastrointestinal (GI) and wound care procedures.

Amy has more than 20 years of experience in specialty care and trauma medicine as a local healthcare provider and military field officer. As a former nurse practitioner in Terre Haute, Amy’s medical experience includes gastroenterology, trauma/triage, occupational health, veteran disability care, and healthcare education.

Amy is a lieutenant colonial in the Kentucky Air National Guard, with a 24-year history that includes trauma medicine and serving as a lead medical instructor.

In her free time, Amy enjoys spending time with family and providing emergency preparedness education to schools, police departments, and first responders.
