About the provider
Chantelle Allen is a certified Family Nurse Practitioner and a board-certified Psychiatric-Mental Health Nurse Practitioner. She provides comprehensive outpatient complex pain medication management, substance use disorders, and psychiatric care for children ages 3 and older, adolescents, and adults through evidence-based evaluation, diagnosis, and medication management.
After years of treating patients with chronic pain, Chantelle expanded her practice in 2026 to include outpatient behavioral health services. Her clinical experience in psychiatry, chronic pain management, and addiction medicine allows her to provide integrated, whole-person care that recognizes the connection between physical and mental health.
In addition to treating chronic pain and substance use disorders, Chantelle treats a broad range of mental health conditions, including anxiety, depression, mood disorders, behavioral concerns, PTSD, eating disorders, and schizophrenia. She has a special interest in evaluating and treating ADHD across the lifespan, incorporating medication management and behavioral interventions.
Since beginning her nursing career in 2010, Chantelle has been dedicated to serving patients throughout the Wabash Valley. She is committed to building trusting relationships and providing compassionate, individualized care that helps patients achieve lasting improvements in their mental health and overall well-being.
Outside of work, Chantelle enjoys spending time with her husband and their four children. She enjoys reading, traveling, fishing, attending concerts and musicals, and cheering on her children at their extracurricular activities.
