Horizon Health Physical Therapy Opens in Indiana

Sycamore Pain & Wellness, a service of Horizon Health, has added physical therapy to its comprehensive healthcare services, reinforcing its commitment to whole-person care and optimal patient outcomes.

The new service — Horizon Health Physical Therapy – is led by Danielle Jena and Ashley Fitzanko, both Doctors of Physical Therapy who are highly respected providers in the Wabash Valley. Together, they provide a wide range of services, including orthopedic and spine care, myofascial release, dry needling, and the Graston Technique – an instrument-assisted soft tissue mobilization that breaks up scar tissue, reduces inflammation, and promotes healing in muscles, tendons, and ligaments.

“Their accomplishments and strong community ties make them valuable additions to our team,” said Dr. Thomas Pendergast, a Terre Haute native who began his pain management practice in 2007. “Adding physical therapy strengthens our holistic, mind-and-body approach to patient care and sets us apart from other pain management practices.”

Danielle Jena, PT, DPT, ATC

Danielle, who is also a certified athletic trainer, has more than 13 years of healthcare experience in the Wabash Valley. Her interests include TMJ physical therapy, a specialized treatment for reducing pain and improving jaw mobility, and the McKenzie Method of Mechanical Diagnosis and Therapy (MDT) for spinal and extremity musculoskeletal disorders.

“I’ve known Dr. Pendergast for many years, and I’m excited to be joining such a talented team that is proud to call the Wabash Valley home,” Danielle said. “Their approach to care is unmatched by addressing all aspects of a person’s health, and physical therapy is an important part of that.”

Danielle earned her Doctor of Physical Therapy degree at the University of Indianapolis. She has a bachelor's degree in athletic training from Indiana State University. She was born and raised in Paris, Illinois, and currently resides in Brazil, Indiana.

Ashley Fitzanko, PT, DPT

Ashley has more than seven years of healthcare experience in the Wabash Valley. Her interests include concussion management and vestibular rehabilitation.

“I’m familiar with the providers at Sycamore Pain & Wellness and I am excited to be part of the team,” she said. “Horizon Health is known in the community, and the patients have great things to say about it.”

Ashley earned her Doctor of Physical Therapy degree and bachelor’s degree in exercise science at Carroll University in Waukesha, Wisconsin. She was born and raised in Hawaii and currently resides in Terre Haute.

Horizon Health Physical Therapy, an extension of Horizon Health Rehabilitation Services in Paris, Illinois, provides physical therapy services to both Sycamore Pain & Wellness patients and other referred patients.
