About the provider
Dr. Cozacov is the chief of surgery at Horizon Health. He is also Adjunct Clinical Assistant Professor of Surgery at Indiana University School of Medicine.
Dr. Cozacov joined Horizon Health in July 2018. He performs a variety of surgeries, including bariatric surgery, laparoscopic hernia repair, bowel surgery, gallbladder removal, and hemorrhoid surgery. Others include treatment for gastroesophageal reflux disease (GERD)/heart burn, cyst and skin lesion removal, port and feeding tube placement, and breast procedures.
Additional Information
Born and raised in Israel, with an Argentinian descent and a Russian last name, Dr. Cozacov has a diverse cultural background. He was discharged with honors after serving as an infantry recon platoon sergeant in the Israel Defense Forces. He then attended medical school in Hungary and completed surgical training at several top US hospitals. He is married with three children.
After medical school, Dr. Cozacov worked as a clinical research fellow in bariatric and minimally invasive surgery at the Cleveland Clinic Florida under the direction of Dr. Raul Rosenthal, an internationally renowned surgeon and pioneer in bariatric surgery. Dr. Rosenthal is chairman of the Cleveland Clinic surgery department and past president of the American Society of Metabolic and Bariatric Surgery.

- Dr. Cozacov has authored and co-authored more than 60 peer-reviewed publications – book chapters, numerous oral presentations, and online videos. Several of his most notable articles address bariatric surgery involving severely obese patients, adolescents, nutrition, hernia repair, and complications.
