Horizon Health Swing Bed Unit

Transitional Care in Edgar County

The Swing Bed Program at Horizon Health is for patients who no longer require acute or skilled hospitalization. This program, sometimes referred to as "private pay," is custodial – meaning patients receive assistance with activities of daily living (ADLs), such as bathing and meals, that do not require a licensed, trained individual.

Services and amenities provided beyond ADLs, include the following:

  • Group activities, led by the activities director
  • 24-hour nursing care with physician onsite
  • Nurses assigned to patients daily
  • Care plan conferences
  • Access to fully equipped and staffed therapy department
  • Monthly physician visits
  • Room-service dining
  • Housekeeping services
  • Constant communication with patients and their families
  • Discharge planning services

Swing Bed Care is sometimes compared to nursing home care. However, there are a number of key differences, which include the following:

  • RN onsite 24/7
  • Physician onsite 24/7
  • Spacious private rooms
  • Private baths
  • Easy access to local physicians

Our Swingbed Program is covered by most long-term care insurances.
