- A Patient Transport Team is available to assist patients to and from vehicles, and throughout the facility. You can call the service at 217-712-9495 (Monday-Friday, 7 am to 5 pm).
- Contact the management of Horizon Health to report a patient safety or quality of care event/concern.
- Report a Patient Safety Concern to the Joint Commission (online form), or by mail or fax.
