We believe everyone plays an important role in helping meet patient needs. We realize that meeting those needs means including family members in the conversation.
Our goal is to bring our organization and communities together when making decisions about patient care. To begin that conversation, the Patient Family Advocacy Council (PFAC) has been created and needs you. The initial time commitment is minimal.
Membership in the PFAC means making a difference in the care that your loved ones receive. The PFAC aims to accomplish the following:
Respect
We value a family’s knowledge of their loved ones; recognize their
authority as decision makers; and respect their choices, family values,
and beliefs.
Collaboration and Partnership
We work together to best meet the needs of families and their loved ones.
Participation
Patients and family members participate in care and care planning. Staff
prepares and supports those patients and families to participate at a
level they choose.
If you are a patient or family member of a patient at Horizon Health, you are eligible to apply to become a member of the Patient Family Advocacy Council.
