Patient Portal Proxy

If you would like to allow another individual to access your patient portal-- such as a parent (of children age 12 or older), legal guardian, or power of attorney--please fill out the online form. Or, print a physical form here. Be sure to include your signature. Bring the form to the registration desk at either Paris Community Hospital or the Paris Clinic with a photo ID to have “family access” granted.
