Discharge Planning begins at the time of admission. Horizon Health provides discharge-planning services to make sure your return home goes as smoothly as possible.
A discharge planner is available to every patient that needs assistance with making arrangements for assisted living, rehab facilities, or long-term facilities. Assistance also can be obtained for:
- Home medical equipment
- Respiratory services
- Home care services
- Transportation information
- Financial assistance information
- Oxygen
- Visiting nurse services
- Senior programs
- Medicare
