Horizon Health has an opportunity for a Family Medicine Physician to join our growing organization.

Hospital Information

We are a critical access hospital and group of rural health clinics, but much larger in scope and size than typical rural organizations. We have cutting-edge technology, new offerings for the communities we serve, strong specialty and primary care support, and still have the small-town approach and values that make our organization so special. In 2024/2025, we completed major renovations to our Emergency Department, added primary care clinics in Tilton and Marshall, Illinois, and expanded physical therapy services into Terre Haute, Indiana.

Family Medicine Practice Information

• We are seeking a compassionate, board-certified or board-eligible Family Medicine Physician to join our growing organization.
• Looking for an individual with excellent communication skills and a patient-centered approach to providing care.
• This role will:
  • Treat acute illnesses, such as bacterial and viral infections, as well as chronic diseases and conditions, such as hypertension, heart disease, or diabetes.
  • Prescribe and administer medication, therapy, and other specialized medical care to treat or prevent illness, disease, or injury.
  • Explain procedures and discuss test results or prescribed treatments with patients.
  • Monitor patients' conditions and progress, and re-evaluate treatments, as necessary.
  • Provide health and wellness education to patients, including diet, physical activity, hygiene, and disease prevention.
  • Participate and work to meet quality measures set for by the facility and accrediting bodies (Joint Commission, IDPH, and CMS regulations).
  • Work collaboratively with our primary care providers (21) and specialists (behavioral health, women’s services, general surgery, orthopedics, spine, pain, dermatology, podiatry, endocrinology, wound care, and cardiology). We have additional support with 13 visiting specialists each month in GI, Rheumatology, Oncology, Nephrology, Cardiology, and Ophthalmology.

Salary range: $200-$400K, commensurate with experience, qualifications, and the potential to contribute to the growth and success of the practice.
