At Horizon Health, we provide high-quality medical care to our friends and neighbors throughout east-central Illinois and west-central Indiana. Since 1970, we have been committed to delivering exceptional healthcare and creating a positive experience for every patient we serve. As an independent rural health system, we offer comprehensive healthcare services close to home, so patients do not need to travel far to access experienced medical providers.
Our health system includes:
- A 25-bed critical access hospital with hospitalist services
- An assisted living facility for memory care patients
- Ambulance services
- Home health services
- Dialysis services
- Six rural health clinics
- Two EZ Care walk-in urgent care clinics
- A wide range of specialty practices, including surgical podiatry, bariatrics, urology, and spine surgery
