Sports Physicals
Horizon Health offers sports physicals at its family medicine clinics and EZ Care locations.
Sports physicals are required annually for student athletes. The visit consists of a physical exam assessing their readiness for athletics. Sports physicals do not take the place of school physicals and no immunizations are provided. Students under age 18 must be accompanied by a parent or legal guardian.

Appointments are not necessary at EZ Care clinics in Paris and Marshall. Hours at both locations are Monday-Friday, from 9 am to 7 pm; and weekends, from 8 am to 4 pm.
